By signing below, I acknowledge and agree to the following terms and conditions for pet care services provided by Hoomane Petcare:

I certify that my pet(s) is/are in good health and up to date on all required vaccinations.
I give permission to Hoomane Petcare to care for my pet(s) during the dates noted above.
I understand that while all reasonable care will be taken, Hoomane Petcare is not liable for any illness, injury, loss, or death of my pet(s) during their stay or care.
I agree to pick up my pet(s) at the agreed-upon time. Late pickups may incur additional charges.
I agree to pay in full for the services rendered before the pickup date unless otherwise arranged.
I will provide all necessary supplies for my pet(s), including food, medications, and comfort items.
I give permission for Hoomane Petcare to take photographs or videos of my pet(s) during their care for promotional or business purposes.
I authorize Hoomane Petcare to seek emergency veterinary care for my pet(s) if necessary and understand that I am responsible for any associated costs.
I agree to leave a payment method with my emergency contact and have one on file with my pet’s regular veterinarian in case of emergency.

Acknowledgment of Understanding:

I acknowledge that I have read and fully understand this waiver and release of liability. I understand that by signing this document, I am giving up certain legal rights, including the right to sue the pet caregivers.
